There are several benign breast conditions like Fibrocystic Changes in which patient is treated conservatively and does not require any surgical intervention. In other cases where there are benign (non-cancerous lumps) we now do either endoscopy or MINIMALLY INVASIVE SURGERY(key hole surgery) in small lumps and in large lumps where removal is required with a formal surgery we give cosmetic scars!
In cases where the lesion is proved to be malignant or cancerous by imaging and pathological investigations, we need to counsel the patient for the treatment options depending on the stage of the disease.
In early cases we give the option of :
Removal of the lump and surrounding margins (followed by Radiotherapy and Hormonal therapy as well as Chemotherapy, depending on the final histopathological report)
Complete removal of the breast), with or without reconstruction at the same time with the patient's own tissues/flaps or Implants. So the patient has a deep sense of psychological satisfaction of the presence of the breast in spite of its removal!
In advanced cases, we might need the help of the Oncophysician to downstage the disease with chemotherapy and then go for surgery (if operable) and radiation. In inoperable cases, there is only palliative treatment like radiation and chemotherapy.
